IMPORTANT INFORMATION ABOUT POLIO VACCINE - FOLLOW UP REQUESTED

• Our records show that [student name] received a dose of oral polio vaccine (OPV) after April 1, 2016. However, that dose(s) does not provide full protection against all polio strains. To ensure protection against all three poliovirus types, children immunized outside of Canada after April 1, 2016 should complete a polio series with an inactivated polio vaccine (IPV).

• Your doctor may not have the inactivated polio vaccine (IPV) in stock. You will need to call them and have them order the vaccine ahead of time.

• Once your child receives the polio vaccine from your doctor, report this to the health unit online at www.smdhu.org/immsonline.

• If you do not have a doctor, you can book an appointment at one of the health unit clinics using our online booking site:https://smdhu.inputhealth.com.
