Notification to Open or Re-open a Recreational Water Facility
This form must be submitted to us at least 14 days before opening a public recreational water facility that has been closed for more than four weeks or a newly constructed/renovated facility.
An inspection must be conducted by one of our public health inspectors prior to a recreational water facility opening to the public following a closure for more than four weeks, new construction or renovations.
By completing this form, you are fulfilling your legal responsibility to notify Public Health of any opening, re-opening or changes to your recreational water facility.
The information on this notification form is collected under the authority of the Health Protection and Promotion Act, R.S.O. 1990 c.H.7.O Section 5 and applicable privacy legislation. This information will be used for delivery of public health services and may be used for statistical purposes.
