Parents and guardians, you are responsible for reporting your child's immunizations to public health to keep records up to date per standards of the Immunization of School Pupils Act (ISPA) and the Child Care and Early Years Act (CCEYA).

Please note that in Ontario health care providers do not automatically report immunizations to their local public health unit. 

There are two easy ways to report immunizations to the health unit:

CANImmunize is a free, secure, mobile app that helps you safely manage and store your family’s vaccination records - all in one convenient place. Think of it as your digital yellow card, right on your phone. Whether at home or on the go, your immunization information is always at your fingertips.

For families in Simcoe Muskoka, the app offers an extra time-saving feature: you can share your records directly with the Simcoe Muskoka District Health Unit (SMDHU). When you update your child’s vaccination details in the app, the health unit’s records are automatically and securely updated too.

Instructions to activate Public Health reporting on Android/iOS

Step 1: Download CANImmunize from Google Play or the Apple App Store
Create an account for yourself and separate records in CANImmunize for each of your family members.
Step 2: Select the record you wish to send to your local Public Health Unit
Once in the record, tap on the 'Connect' tab, and then Select your local Public Health Unit from the list.
Note: If your public health unit is not selectable, you may have entered your postal code incorrectly, or the record is not eligible to be sent because of the age or relationship you set when creating the record.
Step 3: Provide additional information to be sent to the public health unit
Please include the following: Submitter's first and last names, the record's OHIP number, a contact number, and the record's address.
Step 4: Consent
Please read the consent document to share the record with your public health unit and click the ‘I Consent’ button to proceed.

ICON is an online tool that serves as a digital alternative to paper immunization records. It allows you to submit, view, and print your child's immunization records stored with SMDHU, and provides alerts when your child needs or will soon need immunizations.

To submit updates or view your child’s immunization record at SMDHU using ICON, you will need to create an account and set a PIN. 

To do this, you will need one of the following:

  • Your child's Ontario Photo Health Card (their Health Card Number must be on file with SMDHU).

or

  • Your child's Ontario Immunization ID (OIID). The OIID is included in any Notice you may have received from SMDHU.  If you do not have your child's OIID, please call the health unit to request the OIID number at 705-721-7520.  

 

Once you have submitted updated immunization information online, public health staff must process the information before it will appear in your child’s immunization record that you can access online.

We are currently experiencing a high-volume of submissions, so please be patient as we work through them as quickly as we can.

Immunization clinics located in Simcoe Muskoka are for residents of Simcoe County and the District of Muskoka only. If your child attends school in Simcoe Muskoka and has received an immunization notice from us, you can book an appointment at our health unit clinics or with your health care provider.
If you live outside of Simcoe Muskoka, please follow up with your local public health unit or health care provider to make an appointment with them.
