Immunization

Immunization requirements for attendance in licensed child care settings.

We are required under the Child Care and Early Years Act to collect immunization records for infants and children attending child care in all licensed child care settings in Simcoe Muskoka.  Each year we ask parents to provide a complete Childcare Immunization History form as well as a photocopy of their child’s immunization record.

It’s important for parents to share up-to-date immunization records with us to make sure all children are protected. That way, if there is a vaccine preventable disease outbreak, the health unit can determine the children at risk and will contact their parents/guardians.

Children who attend childcare at a school (before and after school care) and are also students have their information collected via the school rather than the childcare centre.

Under the Child Care and Early Years Act parents who do not want their child to be immunized based on religion or conscience, are required to complete a Statement of Conscience or Religious Belief for their child.

If your child cannot be immunized, or you have chosen not to immunize your child, please call us at 1-877-721-7520 ext 8827 to discuss the requirements for your child, and to ensure they are also protected in the event of an outbreak.

Children who can’t be immunized for medical reasons need to fill out a Statement of Medical Exemption form for their child.
